is it possible to transfer my xbox live profile between my xbox 360 slim and modded xbox 360 with LT so i can move my gamerscore points i get from playing modded games

Yes. Your gamerscore stays associated with your profile. So if you load your profile on this box, or that box, or any other box, your gamerscore will be intact.
