I started getting this message recently. I have tried to isolate the timeframe and conditions under which it occurs and what steps necessary to resolve. The pattern apeears to be random, but I'm sure that there's an explanation.

I am on 4.1U, with a Wiikey and Softmod using Larryjle's guide.
I am using configurable USB loader (latest release for IOS 249)
IOS rev 18 (or 19) and Hermes 222/223

The only "nonstandard .iso I am using is the CPS (Copy Protection Stripped) version of NMSB.

I first noticed the error occurring after I ran the Projekt SingStar ABBA .iso. (OK, flames allowed. I'm an old guy, what can I say!?!)

Since then, the problem occurs at random when exiting to the system menu using the Home button. It typically only occurs plaing newer titles. The problem usually clears up after a few reboots. Sometimes I have to let the Wii boot into the Home menu on it's own.

Lately we have avoided playing NMSB (superstition) and instead of exiting to the menu screen, shutting down the Wii after playing games (more superstition).

Any thoughts appreciated.
