i get it when using usb loader gx but not all the time

I also had this frequently until I updated my cIOS version to 13b.
Make sure you have the latest version of usbloader gx. I use configurable usb loader though.

You just remember the last cIOS that you installed and if you can't remember just install which ever one you want now. They install smoothly over each other.
Actually USB Loader GX should tell you which cIOS you have installed in the top right of the credits screen (and if it doesn't then you have a very old revision of USB Loader GX)

Finally as whacker mentioned, the stack dump is likely more of an issue with the revision of USB Loader GX then the cIOS.
