I have one of the oldest Wii's possible, waited in line when they first came out. Oldest drive. 4.2 update.
A Wiikey Fusion with the 1.4 update. I have downloaded 3 different iso's and added them to my micro sdhc via wbfs manager for mac os x.
Going through the press eject twice, use reset button to select game process, I have been able to play every game I've downloaded and added to the SD (Mario Party 8, No More Heroes 2, Naruto Revolution3). ntsc games and I believe no more heroes was PAL
I have downloaded both PAL and ntsc Okami iso's and none work. The closest I have come to success is using the disk utility to burn the NTSC iso to dvd and try that way. The game appears but when I try and play it I get an error.
I have also taken the wii apart and checked all the connections, they are all fine.
Well when I normally launch a game off the sd I select the game, the wii resets and once the wii channel menu loads the game channel shows a disc sitting there and then a few seconds later a disc shoots into the channel from the left and the game I selected appears on the channel. Well with Okami the disc shooting and appearing never happens. It just shows the blank disc. With the DVD I burned okami appeared on the channel but once I tried to play it it said something along the lines of "insert okami game disc"

Any ideas?
For system 4.2 and higher you have to have the same game region as your WII and regionfree patches will not work, do as someone mentioned here, download a new one with the correct region for your WII.

Except a USB loader will bypass that restriction, and he tried both versions anyway.
@ DaiChurch
My first thought was that you're somehow missing an older IOS patch, but okami only needs IOS21 and if you were missing that one then no more heroes wouldn't work either. So I'm not sure.
I would try booting the game through neogamma though. Neogamma is a homebrew app that launches wii game disks for people who don't have modchips, but it has better protection bypasses and can boot games from any region even on a 4.2 wii.

I joined wiiso and downloaded 3 seperate Okami ntsc-u iso's. The last one I downloaded appears to work. I've played about 5 minutes of the game when the wii wouldn't even recognize any of the others.
So I guess it was just a bad iso(s), but i find it hard believe that I had to go through 7 iso's before I found a playable one.

I have a similar problem. I can get to the first loading page of Okami but then the screen turns black and a message appears telling me to eject disk and turn off power but all the buttons refuse to work. I tryed putting a Mario game in and it worked perfectly. I cleaned the disk and everything, unplugging all the cables, turning the power off and so on. I checked nintendo.com but my problem doesn't occur on the problems list. Do you think it has something to do with the xbox plugged into the tv aswell? I don't think somethings wrong with the game because I could play it perfectly about a week ago. One day in Kamui the sound started blanking and the screen lagged then it stopped playing at once. Plus this is a real Okami game I bought from GameTraders with money.
If anyone knows why this is happening, I deffinately would like to know.
