BR-Rip Produces Dozens Of File Fragments

Several movies I've ripped contain dozens of small fragments of the movie, and there's no apparent order or logic to recombining them. Has anyone else encountered this, and how do you piece them all back together? I've managed to figure out one, Die Another Day (James Bond), but trying Nero Vision and Xilisoft Ultimate Converter both crash. Any suggestions? I thought maybe burning them to a BD-RE would be the easiest, then simply ripping that. So frustrated.
