errors in Toast or elsewhere?

I'm pretty new to this, but I've gotten a hold of some of the software that seems to be recommended in these forums. I use MacTheRipper to rip, DVD2oneX to compress, and Toast 6 to burn. I've made some successful copies such as Starship Troopers 2 and Kill Bill Vol. 1. However, some copies I have made have some errors in them. For example, Aqua Teen Hunger Force Season 2 Disc 1 will not play the second episode on the disc, and Starsky & Hutch has no audio. I'm hesitant to make any more copies because so far it seems hit or miss. I'm wondering if I'm doing something wrong or not doing something that I need to be doing to guarantee successfull copies. Can somebody help me out?
