How to format a corrupted memory card

I've read that there is a downloadable file that will allow me format a corrupted memory card if I have swap magic. I have swap magic, but need that file.

Any help is appreciated.

Update: I found out how to format a memory card with Max Drive.
While running the Max Drive software in the PS2 and in the Datel Memory Manager screen, press L1+L2+R1+R2+square and it will bring up an option to format the memory card.
