v1.6 spICE frag problem

I installed the spice(solderless) and everything worked fine for 6 months, last night, it tried to start twice then on the 3rd try it started flashing green and red(frag). If i take the chip off the adapter all 5 lights light up but it gives me error 5( hard drive unlocked). With the chip on 4 lights come on with the correct one off, but the light on the chip is red and it frags/green red after 2 attempted starts. was told that it could be bad connection, so I have removed the adapter 3 times and always get the same thing(frag). Any help would be great, thanks
