I 've heard that the USB External HDD is really slow and doesn't have a very good compatability rate. http://list.ps2hd.com/ says that that USB Extreme works with 1.1% of all PS2 games. check it out...

As for me I have a Seagate 120GB HD (internal) in my old PS2 and I love it... the fast loading time is awsome...I've only had 3 games ever give me any problems..and they were

1. Mafia...the only prob here was no sound, except for the cutscenes...but the game plays fine

2. Red Dead Revolver....the same as Mafia....

3. Evil Dead Fistful of Boomstick...which is totally uncompatible in any mode.....
but every other game so far, has worked for me..
I recommend the INTERNAL Hard drive for anyone wanting to get one for PS2......

Yeah, I'm running the internal HDD solution as well and I'm more than happy with it! :)
I even made some skins for my HDLoader! LOL!

External will always be slower than internal. Its the same with a P.C
I'm happy with my internal HDD and dread the day my ps2 snuff's it and I have to buy a slimline to be honest. As for compatability I find that hD advance is quite good but it seems all forms of boot software have their quirks when it comes to the games they will play. I backed up full spectrum warrior recently. Hd advance would'nt load it so I booted with swap magic. No problem. I have experiances the other way round aswell.

PC Specs: CPU-C2D E8400, M/B-P5N-E SLI, RAM 4Gig OCZ Platinum rev2 ,VGA- Nvidia 8800GT 512MB, HDD- Hitachi Deskstar SATAII 1TB, 250Gig Maxtor IDE HDD.
Corsair HX520 PSU, Liteon Lightscribe DVD Writer, Pioneer BluRay drive.

I dread that day too byngo,I've considered going to the pawn shop and buying a backup PS2 old model , because I had bad luck with the slim.....but as for USB extreme or HD Connect..well, I think that it's great that someone found a way to use the HDD with the slim, but, to me, it kind of seems like a waste seeing how it is only compatable, at this time with something like 25 games or so.
