take sim out press *#06#
the number is 15 numbers
the 1 you provided only has 12

you are correct.
I had omitted the last three digits.
Identity should have read
3553 8800 7636 129.
The service provider remains the same - O2 uk.

Usually you only have to use code 1 or code 7.

3. To get ( w ) press ( * ) 4 times within 1sec
4. To get ( + ) press ( * ) 2 times within1sec
5. Enter the 15 digit code
6. To get ( + ) press ( * ) 2 times within 1sec
7. Enter code number
8. Press ( # ) on your phone

Now your phone is unlocked!
