how do you get rid of the extras and unreferenced material completely using DVDshrink?

Use Reauthor mode.

Unfortunately you lose your menus too.

BUT, who needs menus with 'Movie Only'. I too at one time wished I could retain the menu, but if it only pointed to the main movie and nothing else, I figured what was the point.

If your absolutely adamant about keeping the "Menu", you can use the "Still Image" feature of DVD Shrink to insert a still image instead of the actual video stream.
