I/O errors are Decrypter's way of telling you there's an issue with the drive.
Most often it's caused by poor grade media like Memorex, Piodata, and the list goes on...
A good guide to blank discs manufacturers can be found here:
http://www.digitalfaq.com/media/dvdmedia.htm

Sometimes it's something simple like a fingerprint, which I doubt is your case since you indicate that it's happening often. But better handling of the discs and ensuring they have nothing(not even a spec of dust) on the writing surface when you place it in the tray can be very beneficial.

It can also be a firmware issue as well. Firmware tell the drive how to handle the media it encounters. As new blanks hit the market, firmware can become outdated because the old instructions sometimes don't work with the new discs.
Your firmware is actually a bit old so you might visit here to update...

your drive is the 2100

Another item that can cause issues is more rare, but the lens itself within the drive can get dirty. Using a can of compressed air to blow inside the unit can usually clear it out.

So hopefully you know more about I/O errors and what steps to take to resolve them.
