I kinda lost my slide card, i know, stupid of me... does anyone have any reccomendations on how to make your very own slide card?

tutorial there. follow the size listed on the template, do not just cut out one by tracing it from the picture.

I can sell you a set only a couple of quid for a pack of 2 + postage not really worth the effort making them.
