Looks like you have attempted to unlock the phone more than 5 times.
Your phone is hardlocked. You will have to get it reflashed by a professional for you to be able to use it in other networks :(
I can give you a few recommendations if you like.
Remove sim first,power on the phone.
After the phone boots up, press #
Press * 3 times fast for (p)
Press * 4 times fast for (w)
Press * 2 times fast for (+)
Verify code before pressing the last #.

They are professionals and will help you reflash your phone with new software so u can use it on any network.
This will obviously cost you $$. Not every expensive but worth while.

I think it was the other way around. You were being helped because you hardlocked your own phone. Maybe next time you might slow down and read. Take your phone to a shop in Canada and have it unlocked and if you don't want to spend the money, keep it hardlocked. Not our phone to deal with :)))
