Remove sim first,power on the phone.
After the phone boots up, press #
Press * 3 times fast for (p) \
Press * 4 times fast for (w) IMPORTANT
Press * 2 times fast for (+) /
and the rest of the code---
IMPORTANT!! Verify code before pressing the last #.
Pay attention. You only heve about two seconds.
Note:If nothing happens. Some Phones require that you to insert a non accepted sim, you will see "phone restricted", press UNDO option and enter the code in the restriction box and press ENTER.
"Phone restrictions off", it's unlocked.
"Error code" wrong code.STOP tell me.
"Not allowed" or "Can Not Undo Restriction" it's hard locked. Used up all your free tries!!

I would try to get a new one.
Find an intermittent fault to complain about.
If you get it unlocked, you will be able to use it in the US and some of the Islands.
Cingular in the US charge a fortune to call home!!!
