You are using poor quality disks - TTH02 made by Crap Media Corporation (CMC).
Also you are attempting to burn too fast - Burn process started at 12x (16,620 KB/s)
Suggest you try some quality media e.g. Verbatim etc and try slowing your burn speed to 4x.
