Can someone tell me if my lens needs to be cleaned or replaced.

Won't read Originals, stutters, and on most games it unmatches the sound and gameplay. Like sometime is skips once and there is an explosion on the game and then the sound effect comes 3 secs. after.

Are you on a fat or slim PS2?
Either way go ahead and clean your lens with a laser lens cleaner disc or by opening it up and cleaning the lens with a q-tip and alcohol.
If that doesn't solve anything and you have a fat PS2 look online for a disc read error repair guide (you can find it for free, don't pay those eBay assholes), it could very well be that you just need to tweak the laser a bit.

Most slim ps2's dont get any better when you tweak them from the POT's, the lasers just burn out, so i dont recommend you doing it. But clean the lense with a q-tip and some solution and see if it makes a difference.

Could i use this "Maxell DVD Cleaning Fluid" or should go out and buy some rubbing alcohol.

okay, i use the q-tip and cleaning solution like you said and it fixed the all the skipping and stuttering and it will even read DVD+R now wit no problem but it still wont read originals, i tried disabling the modchip and they still wont start.

Does it not read any type of original disk, or is it just a blue/silver surface disk?

You can re-calibrate the laser to get it to work with dvd disks again, blue surface disks are 700MB or below, they are compact disks, and the silver surface disks, are 4.7gb or below, or 9.4gb of below, you are having problems with dvd disks, so you will have to re-calibrate the potunder the laser thats for dvd's, PM me your email address and i will send it to you.

You will need to open up the drive and remove the rod on which the laser sits on, it isnt difficult, i will send you the guide shortly, and then you can follow the instructions.
