It does not look like the problem is with the original, but with the burn process itself.
Going through your log these are the things that I noticed:
Quote:
Physical memory : 255MB (261616kB)
Free physical memory: 99MB (102280kB)
Memory in use : 60

More is better when it comes to DVD work. Another stick of RAM would be helpful.

Quote:
11:25:59 AM #1 Text 0 File DlgWaitCD.cpp, Line 289
Last possible write address on media: 2298495 (510:46.45, 4489MB)
Last address to be written: -1 ( 0:00.-1, 0MB)
The odd thing here is the location of this line. Recode has taken a look at the media and then at the files to be placed on the media to see if they will fit. Recode has not, at this point, processed your files so it is reporting a "-1" address and 0MB to be written. So as far as I can tell this is where the problem is beginning. It is like you have Recode doing the wrong thing or it has hiccuped at this point in the process. It could be a one time thing. Review your process (try burning one of your first five that worked and select simulate when it comes time to burn.) If it fails this time the problem may be that your Nero is corrupted so a removal and reinstall would be in order.
If it works with a known burnable DVD, try #6 again. Episodic DVDs sometimes have one disc that's a troublemaker and this may be it. I do not see anything in the log that shows this though.

Other things:

Quote:
11:25:59 AM #8 Phase 36 File APIProgress.cpp, Line 278
Burn process started at 6x (8,310 KB/s)
When it comes to write errors, using a slower speed will sometimes help. Give it a try at 4X or slower.

Start of burn timestamp:
Quote:
11:50:59 AM #15 Text 0 File DVDR.cpp, Line 3025
Start write address at LBA 0

Last successful timestamp before the error occurred:
Quote:
1:57:01 PM #113 Text 0 File BurnAtOnceRecorder.cpp, Line 1409
Writing VTS_09_1.VOB at sector 2284839
So it appears your movie was burning away for about 6 minutes before the "KEY_MEDIUM_ERROR" occurred. This tells me that the most probable culprit was your blank DVD.
After the SCSI error Nero says:
"Failed to write 144 sectors at 2285216"
"1:57:28 PM #116 Phase 179 File APIProgress.cpp, Line 278
Writing file to disc failed"

Once that error occurs you then get a "KEY_ILLEGAL_REQUEST" error because the program is requesting to write more data but your SCSI device is saying No Way!

Note that the burn process started back at:
"11:25:59 AM #8 Phase 36 File APIProgress.cpp, Line 278
Burn process started at 6x (8,310 KB/s)".

The subsequent time between this and the actual start of the writing is the time Nero spent "buffering" or preparing the files for burning , doing the two pass thing. Actual burns occur only once. Never twice. Reads are done twice to help calculate compression requirements more accurately.

So thats what I see as I read through your log.
I did not check your burner firmware version because it worked on the first five DVDs. Just make sure to check it once in a while.

The "last address to be written" line is strange but apparently not the problem here because the write process did start. You can look at the time stamps and see the write progress as Nero directs your recorder to place the various video files on sequential addresses on the blank disc.

I did not see any media ID in this log so I don't know what you are using. It is not unusual to find a bad disc in a spindle of lower quality media.

Verbatim 8X DVD-R should be pretty good media...I've used many of them.

Firmware is current as of last month, although I hate to go against franks advice, I'd try an 8X burn...burning too slow can be just as bad as burning too slow. With the advent of quality media and quality burners, higher speeds can be achieved with better results.

Nero API version: 7.2.3.2
This version of Nero may be buggy, try version 7.0.8.2b from here:

http://my.afterdawn.com/locoeng/blog_entry.cfm/280

Use the clean tools{ General Clean Tool, Nero 7 Clean Tool and Registry cleaner), then reboot and install the earlier version...make sure to save your serial number for future installations.

As frank stated, sometimes episodic discs will contain one disc with some added protection on it...the Lost series is known for this. While AnyDVD is great, you might want to try ripping this particular disc with RipIt4Me and then retrying Recode. Here is the download and a guide:

When I burn LOST with Two Passes and Slow Burning turned on it fails, but if I burn it with JUST Slow Burning turned on then it is successful.

I burn Prison Break Discs on Two Passes and Slow Burning with no problems, but when I try LOST it Fails?!?!?!

Why would that be?

Could be the version of DVD Shrink you are using.

Ok, since I can only burn LOST in High Quality Mode with Nero, which would be better to use for quality:

DVD Shrink (with these settings)- Deep Analysis & High Quality Adaptive Error Compensation

OR

Recode 2 (with these settings) - High Quality Mode

Thanks!

@ liquidsli

the "upgraded" versions of Nero are still alot buggy, and not recommended for someone that takes his/her burning seriously. The version 7 that Locoeng recommends is an established "stable" for function. (recommended not only in this thred but others)http://forums.afterdawn.com/thread_view.cfm/386117

have you tried the simple burn to HDD with AnyDVD? (rightclick icon for menu)
