After COMPILE, Lab never creates the 3rd Video (VTS_03...) !

I have 1 VMG menu with 3 links to 3 video sources, no VTS menus. However lab will never create the 3rd video (VTS_03...) nor the menu or VIDEO_TS.IFO. However if I remove the 3rd video all compiles nicely.

Ive been at this for hours. Can anybody help?
