IMEI #: 357064009161188
Country the phone is locked in: USA
Provider the phone is locked to: T-Mobile (midwest)

Note: Do I need to enter code with an unaccepted SIM card or not?

Insert a NON accepted sim card.When it says phone restricted, press "undo."
It will ask for a restriction code,and enter the code in to the box on screen,and then input this code

Press * 4 times fast for (w)

Somehow I did prefer to follow the following instruction ... did work the 1st try.

Not sure why it gave a different code than your proposal.

Unlocking nokia 6101b on T-mobile

I successfully unlocked my nokia 6101b on T-mobile in the U.S. yesterday, here's how I did it...

If you take the battery out, you will find some information on the back of your phone. Mine is RM-77 and the model is 6101b.

1)Download the nokia code generator from unlock.nokiafree.org
2)You need to obtain your IMEI, do this by pressing *#06# -or- you can find it on the back of the phone when the battery is removed.
3) Open the nokia code generator you downloaded in step 1. Select DCT-4, enter your IMEI, and select USA as the country, and T-mobile as the provider. Also select ASIC-2. For the GEN menu, select original.
4) Click calculate and use the 7th code generated.
