When you boot a PS2 game with multi languages, it either presents you with an initial screen where you select the language, or, the game will detect what language to display from your PS2's settings (which presumably will be in English for you).
If the game boots & the language is in French (or another language), then either, its not a multi language game, or, it is multi language, but doesn't include the language you want (English in your case) and the game has booted in its main default language.

Seeing as when you boot the game its in French, then it would be reasonable to assume that either you've downloaded the French only release, or, since you state that the release said it was multi language, then you've downloaded the Multi3 release (from Rome > filename 'rom-cod3') which contains ONLY French, Italian & Spanish.

Its always best to do a bit of research & read any nfo's before wasting time & bandwidth downloading something you can't use.
As far as I'm aware, I don't even think there's been a PAL English language version of this game released. But I could be wrong.
