Burning problem: due to media, program, or cd-rw drive?

Every now and then I get this problem when burning audio cd?s. The tracks are all numbered and divided correctly, but on one or two of the tracks, the time doesn?t start anew at 0:00 at the beginning of the song - rather, it just continues counting from the end of the last song. So if track 3 is 4:26.5033 long, the cd player displays 4:26 as track 4 begins, and clicks over to 4:27 after the first half-second. So if track 4 is exactly 3:00 long, the cd player displays 7:26 as it ends. This is the time as displayed on the LCD of a regular cd player. When I play such discs on my computer, the display on the cd-player program (Win2k) starts over at 0:00 at the beginning of every song.
Anyway, this issue isn?t much of a problem - my cd player will still play the discs. However, I cannot skip to the beginning of the offending track (the one whose time doesn?t re-start at 0:00, such as track 4 in my example above). I can skip to the beginning of tracks before and after that track, but not the track itself.

So not a big deal, but it?s an imperfection. I?m curious if this would be a problem stemming from the cd-r discs I?m using (most recent occurrences are on CMC discs, but has occurred with other media in the past), or from my CD-RW drive (a BTC 12x8x32), or from the burning program (I have Nero 5.x, burnatonce, and NTI CD-maker - the recent errors have only been with burnatonce, but this error isn?t frequent enough to statistically say that it ?only occurs when I?m using burnatonce?).

For the record, I have had other problems with burning audio cd's, such as when the track timing is off (e.g., track 3 is 0:00 and track 4 is 7:26 long. So the disc still plays through just fine, but there is no way, whether the cd is playing in my computer or in a regular cd player, to skip to the true beginning of track 4.
Have also had rare occurrences of each track starting a short amount of time later than it should (which means that track 1's true beginning starts in the lead-in, and is thus unplayable). And this has happened with Nero.
