Most likely it was user error. I hate to be a cold bastard but I find that when crap doesn't work for me there is almost always a cause that could have been avoided if I would have just had more caution.

been there done that as i suffered 1st to almost 3rd degree burns on my right hand. had big blisters behind each of my fingernails, palm of my hand, singed the hair on my hand, arm & head. gunpowder flared under my hand when workinh on a homemade rocket engine.
