DMS4 SE E.Z.I. Pro Test Panel Shows No Lights After Install

I recently installed a DMS4 EZI Pro on a brand new V16 PS2 Slim, and while the PS2 itself still seems to be in fine shape (boots and reads standard discs), the chip shows no signs of functioning (no difference when holding square on a controller while booting), and if I plug in the test panel, a single green light (K) for a short period, then all LEDs go off. I have confirmed that the BIOS and DSP chips are mounted securely, and checked the connections on all of the ribbon cables. (And for those of you familiar with the odd engineering on the test panel, I am using a negative center 9v adapter.) The only thing I'm not sure about is the ground. I currently just have it screwed into one of the GND ports on the mainboard. I've heard some suggestions of soldering it down; does that really make the difference? Has anyone else gotten absolutely no lights on the test panel after an install?

Any suggestions are much appreciated.

EDIT: If I just barely touch the tip of the 9v adapter to the plug on the EZI test panel, all of the lights blink red and green extremely briefly before going out. Does this mean I just have a bad test panel?
