Save Data Between PSP Slim, and PSP

So, i started a FFT file on my old black PSP with the iso form of it, and played for a bit, then when i bought it and played on my PSp slim, it doesn't read the memory file.

Is there any way to convert the savedata from The old format to the slim format?

there's no difference. i had crisis core on my old fat psp, and then switched the memory stick to my psp slim. there were no problems. loaded up right away.

Then what do you suggest is wrong? The save data when saved on the old PSP is like, FFT00800, when the Slim PSP saves it as FFT0063124.
