What firmware have you got?

What filetype is the file?

Where did you get the file from?

What options do you have enabled in recovery mode?

Have you got a UMD in the drive when trying to play a game?
I recommend changing the use no UMD to Sony option.
Also, which kernel is the PSP set at in configuation?

In recovery mode in configuration it shows you what kernel the PSP is currently using, press X to change it

1) Change the kernel and try again
2) Make sure the three options in advanced>advanced are disabled.
3) Make sure a UMD is in PSP and try again
4) Reboot and try again

Go HERE and check the link out.

Make sure both GAME ID and Save ID are the same for all discs.

EXAMPLE:

Wrong way...
EBOOT FOR DISC1 GAME ID# SCUS 94183 SAVE ID# SCUS 92572
EBOOT FOR DISC2 GAME ID# SCUS 94183 SAVE ID# SCUS 95673
EBOOT FOR DISC3 GAME ID# SCUS 94183 SAVE ID# SCUS 96834

Correct way...
EBOOT FOR DISC1 GAME ID# SCUS 94183 SAVE ID# SCUS 92572
EBOOT FOR DISC2 GAME ID# SCUS 94183 SAVE ID# SCUS 92572
EBOOT FOR DISC3 GAME ID# SCUS 94183 SAVE ID# SCUS 92572

Look how the game save ID# is. So look at the Game Save ID# on disc 1 and make sure the game save ID# is the same for disc 2 and 3.
