This can happen if you used poor quality media to make the initial copy with (try using verbatim disks), but, try using DVDfab to rip with instead of shrink, it's better at it with problem disks like yours. Rip to the HDD using DVDFab, then use your favorite app to burn the files to DVD.

DVDFab

A good burning app is imgburn, you can use the build mode to create an .iso image file then use the app to burn the file to disk.

because poor quality media degrades fast using Taiyo Yuden or Verbatim burning between 4x and 8x max on a clean machine without other programs running while burning will fix your problem

My assumption was that you were using shrink and were unsuccessful, that's why I suggested trying to use DVDFab, it's updated and better with the ripping process.

Like I've stated (and so have others), when you use media of questionable quality, this can happen. When you'll have a problem is unknown(too many factors), but it's just a matter of time, so stick to the good stuff and watch you write speed (use 1/2 the rating on the media and you should be ok).

Imgburn will work, but first you'll need to get the video files ripped to your hard drive. You can then use imgburn to build an ISO image file and burn it to DVD using the same app (or you can use NERO).

When you download/install and run DVDFab, the source will be the disk in your drive (choose the VIDEO_TS folder), the destination will be a location on your hard drive. Create a folder on your desktop and give it a name, choose this folder as the destination when using DVDFab (the ripped video files will be placed here).

If the process is successful, you should have the video files on your hard drive, located in the folder you created. At this point you can burn the files to DVD any way you choose, I suggested using Imgburn in build mode to create a .iso image file, then have imgburn burn the fils to DVD. You can also use NERO Burning ROM (using the DVD-Video selection)to burn the files to DVDas well.

If you just can't get a successful rip to your hard drive, well, chances are that no app will help because of the DVD media you used.

Well said.
I also prefer to booktype my backups. I also use those same Verb discs along with my normal TY's.
