Question about NME and patching ISOs for it

My friend has a NME chipped xbox and Ixtreme games do not work for it. I heard you have to use a program called SS merger, which I have. Now When I open it there is a thing on the main screen that lets you load the ISO and then load the ss/pfi files etc and you can do it that way or there is an option under tools that says convert iso to nme or whatever for pre-patched ISOs.

My question is if I have an ISO already that is pre-patched for ixtreme with stealth and what not, do I just use the option under the tools menu to convert/patch the ISO? Also does it over-write the old iso or make a seperate ISO that is patched? thanks for the help guys.
