How to autogen transition text for lots of videos?

I have years worth of video files from my digicam - hundreds of vids. Does anybody have a methodology/software to automatically put the name of the video file with the date of the file as DVD text either before the video plays or as a subtitle? I'm just looking for a way to easily identify/label the videos during the DVD playback

I can easily script the name/date into an overall or individual text file for each of the videos (maybe for subtitle files) - I just haven?t seen software out there can can automatically do what I want - which would be to automatically associate a text file with the video file and place that text in the video/DVD right before the video plays.
