Why Does this Appear Under the "open file" icon

I have a 5G decrpted iso file in my C;users/Ken folder... There are approx 20 other files in the ken folder in addition to the 5G iso file.

When I open shrink and select the "open file" icon i can browse all the 20 folders in the above but it does not show the iso image file.

Why would this be... shouldn't the open file icon reflect the same files as windows explorer... The open file icon had all the file folders except it did not have the iso image file.

Please help me understand

Because that's the way DVD Shrink works. If you want to see the files inside the ISO then you must open it the way already described.
