open imgburn and it will have a box with six choices click make image from disc then the rest is self explanitory. make your .iso and save it wherever you want then burn the .iso you just made
nope same thing do i have to set any settings maybe layer break or something else here the log if it means anything

That has happened to me a few times. If the disc isn't burnt properly then a normal PC drive won't be able to read it, but Xbox drive can (somehow!). Try verifying this game via abgx, if it fails to read the (complete) disc on your PC, then try a different PC/drive.
i have noticed verbatims scratch very easily and will cause a PC to not read it but it will play in 360 fine still. I have a good rip of bad company but the disc has those small hair line scratches on it but they are not deep at all. abgx reports tons of errors from the disc but i know it was a good iso. so i think these DVD+R DL are just easily damaged and that makes them hard to read. I tried making a rip of the bad company i have and it would not rip at all with imgburn just as yours didnt :(
