I've got the same problem with Sonic as well as this.

I did a system update and tried to install Wii 3.3 firmware fix with IOS16-64-v257-prod

but it doesn't load on the homebre channel. It just stays at 0%

I had the same problem with Madworld but i was able to get it to work. I have a usa copy of Madworld and a usa wii with a d2pro. I used the 1.7 utility disk to turn on updates for the usa region only then put Madworld back in. System update came up in the disk channel so i ran it and the game works. I used the 1.7 utility disk again to shut off all updates again just for the hell of it and the game works.
Make sure you only update if your console and game are of the same region.
Hope this helps.

I let my wii update itself through the options to the latest version, and I have a d2pro as well. It's been forever since I needed to bother updating my wii, but I have an NTSC console and an NTSC madworld - if I let it do the system update from the game will it be ok? Or should I get this 1.7 utility disk that smokestac is talking about?

Let it do the system update from the game as long as you are sure that the game and console are of the same region.
If you have a d2pro it would be a good idea go to d2pro.com and get the 1.7 utility disk so that you can update the d2pro to the latest firmware.
