Hey man, I was doing the exact same thing you were doing... following the guide here, downloading the needed files, blah blah blah and somehow, even though I followed every step word-for-word, I would ALWAYS get an output video file that has an out of synch audio.
The solution? Go get Eazy VCD.
Trust me, go get that and follow the guide guide here and believe me, in less than FIVE MINUTES you will be ripping a DVD and having an output file (whether mpeg or cue/bin) that is 100% right EVERY SINGLE DAMN TIME!
I wish I could re-post this reply to ALL the guys having problems ripping DVDs here because really, this is SIMPLY THE EASIEST, FASTEST AND SUREST way of ripping DVDs withoug doing much work! You just tell it what chapter you want to rip, select the audio and the screen size, the CD size you're using, etc. and voila! No hassles, no problems.
Now, the guy who made this program was asking for just two bucks to help him pay host his site and you know what I did? I was so happy to have found this program and so glad that it was so easy and I've ripped THREE movies already using it (a 90 minute movie takes me anywhere between 5 - 7 hours to rip) that instead of donating just two bucks, I PAYPAL'D THE GUY 100 HUNDRED DOLLARS! His program saved me HOURS of ripping DVDs.
Again, I can not emphasize enough how easy and fast it is to use this program. All I ask is that if you successfully ripped a DVD using this, tell somebody here to use this program or copy/paste my reply. I'm not even going to bother editing this. I'm basically rambling right now.
Oh well, I gotta rip more DVDs...

PS. Remember to download the main program and the patch found on the MAIN PAGE of his site and go to the downloads section and get the ASPI things. Then install all three and good ripping times, man.
