At work now so I can't give you exact details.
I use JF .51 via serial cable, spear, extractor, to get the key. I than use another program to prepare the custom firmware. Than it's off to a usb flash drive to boot into basic dos and run another program that does the flashing.
I've had zero issues with this setup over several flashes. If ya care, I will reply later with the exact names and steps I use.

I use a 360 Xtractor Pro. I also use the serial port instead of usb. And actually, I don't use iPrep at all for a liteon but still very simple.
Take apart the 360 and take out the drive. Open the bottom of the drive case so you see the board.
Turn your PC off and connect the Pro to a power spot, serial cable, the power for the liteon, and the spear. I never remember which way the spear cable goes so I just put it one way, turn the PC on, Pro on and see if the green light comes on the spear. If not, flip the cable on the spear or the Pro until you get a green light on the spear. Once I got the light, I turn the PC back off and connect a sata cable to the liteon. I use my mobo's sata port.
So everything should be connected. At this point I have the Pro turned off. I turn PC on and wait till its in Windows. I turn on Pro, eject tray, turn off Pro. I move the tray in:

I don't think it has to be exact spot. I then turn the Pro back on and the tray should remain in place.
Open JF, mine is 1.51, and go to the dvdkey32 tab. Go thru the I/O ports until you see the liteon:

I only have com 1 so no issue there. So with the right I/O, you want to touch the spear to the correct hole:

The spear should now have the blue light on. Click the get key, create dummy blah blah button and it should start saying reading key or such unti it pop ups a save window. At this time you can put the spear down and save the files, it will ask you to save four files in a row. Take note which port your drive is on! the last for digits are very important. For example, mine is B480.
Great, now we got the files we need. You can now put the tray back in and turn off the liteon and close JF.
I create a simple folder like ! and put the four files in there. Next you want to get Firmtool 1.3.1 at:

Copy the exe file to the folder with the four files. You will now then copy the ixtreme 1.6 into the same folder. Personally I name the file ix16.bin.
Open a command prompt window and go to the folder with the files. Type the command: firmtool.exe dummy.bin ix16.bin
This should than give you a green text success blah blah. Now the ix16.bin file is the custom firmware ready to be flashed.
To make it faster, here is the files you need for a simple usb dos boot plus the programs we need for a flash:
http://www.MegaShare.com/1104235
Delete anything on the usb and extract the file on there. Now, put the ix16.bin that we just got made on the usb.
Turn off PC. For ease, I unplug every drive in my PC except the liteon, naturally. Turn PC on and boot to usb. Don't worry if it says bad command blah blah and than you got a C prompt. Turn on the Pro.
Type the command: l-o-eras.exe sataport
You remember when I said to take note of the port, this is where you enter the four digits. So mine would be l-o-eras.exe b480. Now, you have to repeat this command until you get a result that ends in 72. Generally it says xx80. Try the command 10 times, if you still getting 80, turn off the Pro, wait wait, turn it back on. Try the command again, generally it takes 12-15 trys.
So, after it gave us a xx72, type the command:
dosflash.exe
To will then list every drive it can detect. It will ask you to select a drive. If you smart, you should only have the liteon connected so enter that number. It will than ask you what do you want to do. Hit W. At this point, it may or may not give you an error saying 'unknown flash chip' and dump you back to a C prompt. Turn off the Pro, wait wait, turn it back on and repeat from the dosflash.exe command. So pick the drive number again and than W. At this point, it will ask you the name of the firmware file. For me, it would be ix16.bin and than it shall do its thing and finally give you a sum file and the C prompt. Your done.
Wow, thats a lot of text. I should clean that up but here ya go!
