Download the installer package (inc new forwarder channel) from the google code page (Google finds it if you search gx loader)
There have been a LOT of upgrades to both cios's and gx loader.
I would find a pc program called modmii, run the wizard on your pc and it will download all the new cios WAD's you need (treat your wii as a virgin one)
It will also install ios 58 and advise you to reinstall the HBC if it is not already running on this ios (a full guide is included)
Then install the gx forwarder wad and put the new boot.dol in the folder (NOTE:- the position of all the config files and disc files has moved, either move them yourselves to the GX folder in the apps folder - or redownload them)
Once done - there is no more alt.dol, and ios reload games all work (C.O.D 3 and Sam and Max etc).
Also if you find the Beta thread the latest beta releases also enable saves to automatically put on to the SD rather than the Wii menu

I guess kittymat figured that your cios files are outdated and updating might makr things run smooth for games that won't work on your cios. Your on rev 14 and last i checked its up to rev 19. but you could use the same wad manager program to uninstall the forwarder wad first. Then reinstall the forwarder wad and see if that works. But if it really did update then you might need an updated forwarder. But remember to uninstall the forwarder wads first before you install a new one. And don't accidently uninstall something else by mistake :)

Actually the whole process will take very little time. Modmii will download all ios's, cios's and a detailed guide (and the wad installer etc), it will also download ios 58 (for homebrew channel and the latest forwarder).
Lots of new games need a cios with base 56 (this will run 99% of games without issue)
Some require a cios with base 57, modmii will install 249 and 250 with base 56 and 57 respectively.
The latest GX will not run unless you have rev17 or above

Do the modmii update first, if you have chosen not to update your Wii system menu from 4.2 (recommended option) then you have the hack and priiloader already.
Start the guide (found in the copy to sd folder)after the install of the homebrew channel.
Install the wads, then make sure that you reinstall the homebrew channel (covered in the guide by "upside down homebrew channel"), this ensures that it is running on the correct ios.
In the installer package for GX you need the boot.dol file, this needs to be placed in the GX folder in the apps folder on your SD, (ignore the elf if included, as this is no longer needed). The forwarder WAD (the GX specific one - the one modmii downloaded is a generic one)can be installed by adding it to your wad folder on the sd (delete the others as they are no longer needed) and installing it using the relevant part of the guide.
DO NOT UNINSTALL ANY WADS - if you want to get rid of the old channels / forwarders use the Wii system menu to do this, its a lot safer.
Once you have GX up and running you will need to play with it a bit to see all the addidtional options, you can lock each part of GX (inc homebrew channel) via the parental lock. Catagories, you can put games into catagories, I have one catagory for each child with their favourite games in each, so they each can get to their own favourites easily.
If the homebrew channel update is causing issue,
1) try to redownload it via moddmii and reinstall
2) connect your wii to your wifi (if you have it and use the update function in the homebrew channel)

Unzip the all in one package, there are two available one for cios 249 (prefered) and one for 222.
You will get two folders, one says Apps, inside this is the usbloader_gx, on your current SD card you should have an apps folder, copy the usbloader_gx from the pc to your SD (overwriting your current one - this updates the GX loader)
Next you need to install the latest forwarder, this will only work if you have installed ios58 from the modmii package, open the WAD folder on your pc and you will see a wad file named "USB Loader GX-UNEO_Forwarder_3_0_AHBPROT.WAD" this is the wad you need to install. Use the modmii package to install it (delete all the WAD's in the WAD folder on the SD card, add this one and install it the same as you did the other WAD's)

Ok! I'll give it a go in a day or two when I get the time. I wonder if, becaause Modmii did not download Bannerbomb and because I can't locate it on the web anymore, I can skip this (because I already have HBC installed) and just update HBC I already have installed. So many qn's! Will let you know how I go. Can I just update HBC then install the resot of IOS's please let me know how to update HBC if I can do that. I checked I am running version 1.0.5 & IOS61 V21.29.

I run my hbc off of ios36. And most programs need hbc 1.0.7 or higher. The newest is hbc 1.0.8 version. You can get it here: http://bootmii.org/download
There is a read me.txt file in it to tell you how to install.

The easiest way would be to let modmii do it(as you already have them downloaded, copy everything in the copy to SD folder to your SD card and use the WAD installer to install all the WAD's. You can tell by their titles which wads you are going to be installing. Most important ios58, 249, 250 (updated ones from modmii) Also add into this folder the forwarder WAD you downloaded from the GX package
All the wads will be in the WAD folder so you can check you have them all on your pc prior to starting, if you want to you can delete all the other wads, they are not required for homebrew but add brick protection etc.
If you install the HBC and there is no ios58 then it will default to ios61, however it will not swap to ios58 unless you reinstall it so better to install ios 58 then reinstall the HBC
All covered in the guide that is also in the copy to sd folder

Thanks! I tried to run the package Modmii downloaded but it skipped over the Bannerbomb file needed which is the first step in the process so I can't do this. I tried to go to the Bannerbomb link Modmii provided (in case the file did not download) and the files have been removed. I can't find it anywhere on the new that I know is safe (I can dl from a fileshare source but I don't have any feedback on how clean the files are etc).

I already told you where to get it. The hackmii installer is the bannerbomb. You can place the hackmii folder into your apps folder on your sd card and run it from the hbc.
