Adding custom subtitles to DVD

With which tools I can add custom subtitles to a DVD5?

Some instructions also on how to do this, would be appreciated. If possible, someone with extra time on his/her hands could write up a howto on this subject?

Subtitle timings, format and such aren't an issue, since there's lots of subtitle tools available with which format, framerate and timings can be adjusted for the movie.
