DVD not playing in DVD player after putting my own subs..?

I purchase a DVD, but i really want to add my own subtitle. So i got a new subs, then try to combine it with the main movie. Because i am not sure how to do it at first, so i used tons of programs to convert/combine/whatever.. back and forth, then it comes out pretty good result in my computer. It works perfectly fine on my pc. But after I burn it to a new DVD, its not playing on a stand-alone DVD player. I'd sure the DVD is not scratch.

Software i had used include: DVD Shrink 3.2 --> SubtitleCreator --> Subtitle Workshop --> SupRemap--> Nero

Anyone knows what is wrong in my precedure? Any better suggestion or shortcut?

If anyone need further info, please note. I really do appreciate any help. Thanks.
