Unlock my Nokia 3120!!!! HELP!!!

Hi guys and gals

Please help me, been trying to unlock my Nokia 3120 and having no luck!
Heres the details

Nokia 3120 Type RH-19
IMEI 357052009799966
T-Mobile phone but I want to use a Vodaphone sim card

Am I meant to have the vodaphone sim in the phone when I type the code in? Tried one of these online calculators but could not make head nor tail of it and got code error 4 times! What happens after the 5th?

Any help would be great,
