I need help to unlock my Nokia 6101 from T-Mobile (USA).

Press #
Press * 3 times fast for (p)
Press * 4 times fast for (w)
Press * 2 times fast for (+)
Verify code before pressing the last #.
If it still says Restriction on after you have entered the code, then the code is incorrect.
If it says Restriction Off, then the phone is unlocked.

what did it say when you entered the code?
have you ever tried other codes prior to the ones that i've given to you. if so, how many codes have you tried. if you have tried more than 5 times then the phone is hardlocked. take it to a shop and get it unlocked by cable/software combo.
