do a search, there are hundreds of sites that sell the Swap Magic discs. You will also need an aftermarket flip top to go with it or the discs will do you no good.
With a chunky PS2 you could also make a slidecard instead but it is a pain and you will regret not getting a fliptop.
With the slim PS2 you could get a slimtool/ghost switch/magic switch
instead of a fliptop. All of these serve the same purpose which is to be able to open the PS2 to swap the discs without it resetting.
