Swap magic or mod chip?

What brand of media (discs) are you using?

Did you burn the ISO as an image rather than data?

I assume you mean the PAL version, the NTSC version isn't available as far as I know. What's the Media ID of the discs you're using?

91. Q. What is the Media ID of a DVD and how can I find it?
A.The Media ID of a DVD is the true measure of it's quality. It contains prerecorded info about the disc's manufacturer. The quality of the disc will vary depending on the media code. The media code isn't visible on the disc or the packaging. To check the media ID, Open DVD Decrypter, insert your disc, and check what it says under Media ID or Manufacturer ID. You can also use a program called DVD Identifier which is very useful. You can find it here: http://www.afterdawn.com/software/video_software/dvd_tools/dvd_id...
Compare the Media ID to this link: http://www.digitalfaq.com/media/dvdmedia.htm

The same exact thing happend to me because i tried converting from PAL to NTSC but it turns out that the ISO was already converted to NTSC by the person who uploaded it so just burn the regular ISO you downloaded and if you are Using swap Magic use the NTSC video option and press Sqaure and it should boot up fine.
