Rebuiliding doesn't work (unless there's a dummy file I'm missing), and extracting content in IsoBuster (which so far has worked for *every* game I've tried) doesn't work, using SwapMagic 3. Any help?

I rented MLB 06 The Show from Blockbuster. I extracted the files with ISO buster and rebuilt it with CDvDGen 2.0 and it works fine for me. I'm using Swap Magic 3.6 with a slimline ps2. Did you download the .iso or get it from a retail copy?
It helps as far as what I need to put in the relevant boxes, and then it all falls apart. CDvDGen only seems to export the .iml and not the .ims - whether this is because MLB 06 is single layer (like CDvDGen would suggest) or not, I don't know. Regardless, I'm still stuck because iml2iso won't convert the file without the .ims.
