To create a patch for a game head to ps2dev.org and grab the SDK for the ps2 and build from there assuming you know some programming skills.

That is how you " Create " a patch.

If you want to " Apply " a patch then get PPF-O-Matic.
Or something similar.

having a problem with the submit button?
What they do can differ. Most of them save you the trouble of having to do things manually like rebuilding a game or removing dummy files if needed. You just apply the patch to the ISO or game file that it states and it will do all the work for you.

Having the original or not has nothing to do with patching it.
If you use Swap Magic, then some games need to either have dummy files removed or just have to be rebuilt to work.
Even if you are making a copy from the original disc. It is one of the drawbacks of using Swap Magic.
This can all be done without getting a patch, you can do all of this manually if you wanted to. It would take you less time to just rebuild it manually then to create a patch to do it.
What game is it that you are thinking needs a patch?
