HELP! Microsoft Word!

Hello! I am having a lot of difficulty finding an appropriate forum to get help with a problem that involves Microsoft Word. I'll describe my problem and hope someone can give me some advice on how to proceed. I am an inter-city bus driver for Trailways (GO BIG RED)in Syracuse NY. I have to turn in a TRIP ENVELOPE measuring 6 inches by 9 inches. The front of this envelope is formated; I have to fill in information like run #, date, from, to, leave time, arrive time, # of tickets inclosed, # of cash passengers, total # of passengers, # of logs inclosed. I want to scan the front of the envelope as a document, bring it up later, type in the information, place a clean envelope into my printer and print it. I have gone to several forums and received only one response telling me I have to scan a blank envelope to file, then open Word and place this scanned file into "normal.dot". Doesn't work at all. I tried wading through Microsoft's voluminous database for the answer but came up empty. Can someone please help? Thanks!
