How do I add my own subtitles to dvd I just backed up? Please help

My problem is this, I just backed up final destination and I want to be able to add my own (Polish) subtitles top the video and burn it to dvdr, how do I do this.

I know that I first need to convert the subtitles to a valid format, what utility is best for it? and how do I add those subtitles? Please help, I am clue less
