You need to run the icons through an icon converter.

Simple thing to do: Go into Recovery -> Advanced -> Toggle USB (flash0)
When the screen comes up on your computer copy all the files onto a safe place on your computer.
If you brick your PSP, go into Recovery at startup and Toggle flash0, get the files you stored on your computer and re-write them over your bricked PSP's flash0, you will now have a working PSP again.
I've done this many many times with no problems what-so-ever.
Also, before i can help, which icon converter are you using?

Well, firstly, i do it all manually, that means going into flash0 to change my files, i never use a flasher program.
A good place to get custom themes etc is http://evolvexmb.com
But you can find them all over...
The first thing i suggest you do is backup your flash0 to your computer (see my last post) and go onto the website i mentioned above, you'll have to create an account to download things, but it's worth it.
So, download one of the themes that is meant for 3.40 OE-A and copy the files over manually to your flash0, if it works then you can use the standard themes (you don't need to convert them to Jap like some people)
Meanwhile i'll track down the icon converter you'll need to use on older themes.

There are lot's of ways to free up a little more flash space to be able to fit in the larger themes, you can get a few homebrew apps which will get rid of some of the files which you don't need.
One thing i'd suggest is to delete your "dic" folder, it will have no side effects. But back it up just in case.

From coldboot? I've only ever seen a couple of these, and not sure which file it is. If you want to add actual videos when you turn your psp on then search for a program called cintro.

Well, you could try and find a site that has a whole section for them or something, but just look for the more complex themes, they usually have a different opening come with them, so you can try out the ones from different themes.
