Downloaded short .exe - BSOD every time I open a program

Hey all, been here helpin out a while back, but now I found something I cant even fix!

I downloaded a small exe (30kb) - which was obviously a virus and I then got spyware messages popping up saying what they say - it was a program that installed itself onto my computer. Got rid of it in safemode...

However, now when I open itunes, tell CCleaner to clean, pretty much everything besides open the internet I get a BSOD. System restore does not fix the problem...

Any help would be greatly appreciated.
