Best DVD to Premiere?

Greets,

I have been banging my head against a wall trying to rip DVD's into a format which can be edited in premiere, and then exported into something which is not pixellated to death.

I have learned that most of the DVD rippers out there (No1, AoA, Xilisoft) all seem to rip to XviD or DivX; both of which are known to not be importable into Premiere (they produce an audio rate error, which, when searched).

The closest I have found so far is that River Past makes a Video Cleaner program which can read a DVD (if you install the GPL MPEG 1/2 decoder) and output it as AVI/DV (or many other formats).

However, while the AVI/DV it converts to is pretty good, the further export is horribly pixellated.

I realize each step can possibly involve further compression, but this is all digital. Reading a DVD, editing, and exporting, into something which looks nearly as good as DVD should be possible, shouldn't it?

Can anyone with experience doing this (such as with their wedding video, vacation video, etc. - anything which has come on a DVD), PLEASE PLEASE PLEASE weigh in on what programs and steps I could take to be able to rip, import into premiere, and then export, into something usable.
