HELP! USB flash drive asking to insert a disk

I was trying to create bootable USB flash memory drive for vista. My usb drive not working anymore. here is my problem.

Run CMD.EXE and type the following command. Note: This set of commands assumes that the USB flash drive is addressed as ?disk 1″. Double check that by doing a list of the disks (type ?list disk?) before cleaning it).

1.diskpart
2.select disk 1
3.clean
4.create partition primary
5.select partition 1
6.active
7.format fs=fat32
8.assign
9.exit

after I enter that in the CMD.EXE, I try to copy vista to the flash drive by "xcopy h:\*.* /s/e/f e:\" my usb drive is at H and my vista is at E. I tried typing in different order for E and H drive.no luck. It showed invalid drive specification 0 file<s> copied. I can't even format that usb drive anymore. It asking me to insert a disk into removeable disk (H:) when i double clicked to that usb drive.

I don't know what to do to that usb drive I can't even do anything to make it work. Please help me out.
