What brand dvd-r are you using? Some good brands are Ritek, Fujifilm, and Memorex. What version ps2 do you have? If you have a version 1-4 and you are using dvd+r that could be the problem because versions 1-3 can't read dvd+r and some version 4's can't read it either or have major difficulty doing so.

A backup of a backup shouldn't be an issue. DVD-R is the correct media to be using on a v4 PS2 but the unbranded media could be the problem. Trying using a higher quality media such as Verbatim and burn at 1x to see if this solves your problem.

PS2 - v8
Mod - Magic V
Burner - CenDyne (Pioneer) DVR-A05
Media - Anything Cheap

Gamecube w/Broadband Adapter
Phantasy Star Online
Animal Crossing Loader v1.4
Phantasy Star Uploader v1.1
Gamecube ISO Ripping Server v1.0.0.29

As Icemonkey sez, making a backup of a backup ain't a problem, it's surely down to the media mate. By the way, don't do the copy on-the-fly, make an image on your HDD first, then burn it off. Try using DVD Decrypter, free to download off this site, MODE-->ISO Read, ISO Write.
Good luck
